8/11/07  Equine Helmet Waiver Request Form  You can either use the electronic form or you can find one located in the Pan Abode building on the fairgrounds.  A KRSF, Inc board member must witness both the parents and child's signature on the form.  The following is a list of KRSF, Inc board members:
